Overview
A half day programme for attendees to fully understand the skills and mindset necessary to use Social Prescribing confidently and communicate with clarity. Also to understand and embrace change to build new behaviours, be resilient and maintain a positive mindset throughout the whole process. The course will enable attendees to build the skills and confidence to enable them to build empathy to manage conversations and actively listen to understand the most appropriate response and resource for every patient.Who is Social Prescribing aimed at?

By the end of the course delegates will be able to:-
- Define social prescribing, the history, the principles of personalised care and the role of the social prescribing worker
- Build trust and rapport to influence positively and confidently
- Recognise symptoms and appropriately deal with the patient for social prescribing
- Understanding what support is available, what tools are available
- Understand the importance of body language and how to utilise it positively
- Active listening and Confidence raising techniques in the workplace to ensure appropriate signposting first time for every patient
- Being able to develop personalised care support plans effectively through a “what matters to me” conversation
- Sensitivity to different cultures, beliefs and differences to ensure each patient is empathetically supported
- Knowing how to co-design a social prescribing offer with your local community
- Manage the conversation by understanding their reasoning with empathy
- Remain calm and professional when faced with any form of conflict from low level
- Reflect, feedback and improve the signposting process for all practices including self-care to increase confidence
